There are two pieces of evidence which shows that the Terror Mask has knowledge of the fact he's in a fictional universe and in a video game. The first piece of dialogue has him state the game's Title. Another one was the following: "See, now that's the **** that gave this an M Rating."
Lastly, if you die and don't hit continue quick enough, he'll ask, "Can you hurry up and pick 'Continue'?"
The instruction manual artwork is an homage to the Japanese box art of Splatterhouse 2 on the Sega Genesis. The only differences are Rick, Jennifer, the Terror Mask, and the Boreworm next to Rick all have different designs, and far more blood is on Rick. Aside from that, the art is nearly replicated.
Despite being mentioned in previous games in the franchise, this is the only game in which Dr. West was seen, and players got a glimpse of what he looked like.
