  • Part of the opening introduction includes a scene where Gillian and Jamie are having a conversation on a building rooftop. This scene was included as a comic in the instruction manuals of both the PC Engine and Sega CD version, but the Sega CD version is the only version of the game to contain this scene in the game itself.

  • There is an urban myth that typing in the word "KONAMI" into the Jordan computer will uncensor the game. However, it has been ascertained that this code does nothing, despite making a jingle noise when the code is entered.

  • In the PC Engine version of the game, investigating the black market store enough times will uncover a sex tape. If you purchase this tape, a scene will commence where Gillian watches the video and gets excited, while Metal Gear freaks out by what he sees. This scene was cut from the Sega CD version of the game but the video tape can still be found when investigating the store.

  • In all Japanese versions of the game, Katrina's bra can be found outside of Gillian's shower stall while she is in the shower. You can even examine and "interact" with the bra as well. This was omitted from the Sega CD version as it was considered too risque at the time, and it made it too obvious that Katrina was in the shower.

  • In order to avoid copyright infringement laws, the snatchers in the Sega CD version of the game were given green eyes and their body parts were darkened. In the PC Engine version of the game they closely resemble terminator machines, complete with red eyes.

  • Queens hospital was called "Queen" hospital in all Japanese versions of the game. The reason for this small change is because Gillian comments that the word in the picture looks curved and may continue outside the edge of the image.

  • In all Japanese versions of the game, Gillian plays a game of Janken, which is the equivalent of Rock, Paper, Scissors with Metal Gear and bounty hunter Random Hajile. This scene was omitted from the Sega CD version as the outcome is the same whether Gillian wins or loses.

  • A small number of changes were made to comply with censorship standards in North America. Katrina's age was changed from 14 to 18, Lisa Nielsen's breast is no longer exposed after she is killed, the shot of Katrina in the shower is slightly masked by Gillian looking in, and the player does not have the option to show genitalia when Katrina asks Gillian for his ID.
