Game Modes / Unlockables
There are a variety of Game Modes and Unlockables in the game. Here you'll find all of them with their conditions to unlock.
Weapons, Abilities, and Costumes
Extra Kalina Ann
In Mission 11, after dropping all sections of the building into place, return to the tower near the middle. On the top floor, there will be a Gold Orb. On the bottom floor, one of the windows has been blown out. Go through the hole and you'll find a room. Towards the back of the room will be a prompt ("???") that will allow you to pick this up. It will not be visible on the floor. It's the only weapon in the game not collected through the main story.
There's another Kalina Ann that you collect at the end of Mission 17. If you collect both, you'll have access to the Double Kalina Ann. This is essentially Dante wielding both of them at the same time for even more damage. The Double Kalina Ann has its own skills (though many are related to regular Kalina Ann skills).
Recovery Provocation
This is unlocked for Nero by defeating one full round of enemies during the credits sequence that play after the end of the stories. It requires completing one round for Nero and one round for Dante in the same sequence. The difficulty of this (including enemy types) scales with difficulty. All combat features including DT and Devil Breakers are available, just not visible.
When unlocked, Nero learns a new provocation (L3 + R1/RB + Taunt). It can only be used once per level, but Nero will eat gum and recover health.
Special Provocation
Each character has a Special Provocation skill that is unlocked by using them to complete the Bloody Palace. Once unlocked, you'll be able to use it immediately. This mostly just provides fun cosmetic changes, such as forcing Nero's hood up.
Dante's Irregular Full Custom Skill
This skill is unlocked after completing the game on Son of Sparda and is added to the Abilities section of Dante's shop. It costs 100 Red Orbs to purchase and expands his weapon slots to have enough room for every weapon.
This allows you to use Rebellion, Sparda, and Devil Sword Dante all in the same loadout, but this adds a lot of redundancy, potentially adding unneeded fluff that slows effective weapon swapping.
Difficulties and Game Modes
Before tackling how to unlock each mode, here is a list of differences so you can decide if you want to go through the trouble. It's tough, but playing at the higher difficulties is rewarding experience!
Differences Between Difficulties
The most noticeable difference between difficulties will be damage values. Enemy health does not change per difficulty, but damage dealt to enemies (and to you) DOES scale.

This is just the most topical difference in difficulty, and it can have a huge impact. See below for a breakdown of the major difficulty-wide differences.

Son of Sparda Difficulty
Son of Sparda (SOS) difficulty is unlocked upon completing the game on Devil Hunter difficulty.
Dante Must Die Difficulty
Dante Must Die (DMD) difficulty is unlocked upon completing the game on Son of Sparda difficulty.
Heaven or Hell
Heaven or Hell (HoH) is unlocked by completing the game on Dante Must Die.
Hell and Hell
Hell and Hell (HaH) is unlocked by completing the game on Dante Must Die.
