I think I'll stop keeping track now, because stuff just got complicated. Not sure if cards were a relevant reward for policies in chapter 1, but they seem to be now. And popularity changes are now larger than 5%, but not all the same.

Chapter indexing isn't efficient, as the order at which issues appear is partially random, or anyway dependant on certain conditions.

One of these conditions are faction main storylines (ex: imp megabomb) that you slipped in your list. Generic political issues always have 3 councillors for and 2 against. The result might range from only +/-5% with them to some other minor adjustments (ex: going against the Undead Divines often gives you a small luck malus, etc.).
However, when you reach 85% favor with a faction (which might or might not happen in act II), you'll be proposed the first of 4-5 (sometimes plus variants) BIG TOPICS. In this case only the proponent is for the motion, the other 4 will be against. The rewards (and penalties) are much higher: the first motion will usually still be relatively minor (-2% with the other races, and a somewhat common bonus/malus combo), but after that you usually get both big modifiers AND faction cards (AKA permanent cards). Permanent cards are, as the name says, permanent game modifiers, often very useful (the elves give entrenchment cards, the undead ones will make creating units in RTS battles MUCH faster and so on).

If you refuse one of these major topics you can still be offered the subsequent ones when you further increase your standing with the faction back to 85/90/95% (usually variants; EX: if you accept all major topics by the undead the last one will be a proposal for the institution of slavery, but if you refused the previous one the topic will be about instituting the Inquisition instead). Refusing these topics will also give you an increasing major boost with the other four races, meaning that it's rather efficient to accept the first ones (getting -2 and -5 and sometimes -10 with the other races) and then refuse the last to get back a whopping +15% with everyone else, so you can easily get to their "main quests" too.

The result of this mechanic is a bit silly sometimes: with the undead you practically make the kingdom a fanatic religious state, but voting no to blatant slavery makes everyone jolly again. The same goes for the lizards, as you can make every other councilor almost reach a nervous breakdown by having a referendum, and a democratic vote (that you lose), and wrap everything back as it was by simply refusing to abdicate in the end.

*Small correction and addition: generic diplomatic issues always have a 3vs2 ratio of councillors. I worded it badly: they can both be 3 for / 2 against or 2 for / 3 against. Anyway, when you see it's 4 against 1, then it's a faction major topic.
Also, you can check in the diplo stats menu to see when you'll be proposed the next one by hovering the mouse over the relation bar.

I wonder if the effects are the same at all game difficulty settings?

I like what you got so far though.
I'm glad your opinion differs from mine. What a boring place this world would be if it didn't.
