  3. How did the Japanese voices handle the different regional accents of Ivalice?

My understanding is that the English localization wanted to get creative with the different regional dialects of Ivalice. Dalmascans have American accents, Archadians have British accents, Bhujerbans have Sri Lankan accents, Viera have Icelandic accents, and the one Rozarrian we meet in the game has a Spanish/Russian accent that sounds kind of like Borat. But did the Japanese voices all speak the standard Japanese dialect with no apparent regional accents, and it's only in English that Square Enix sought to differentiate every group?

From what I remember, most distinctions in the japanese dub are done with the way they talk, not accents.
Royalty and that sort of people talk with an "older" and more formal japanese, while younger people talk with a more casual and modern japanese, not so much regional accents.
