How do the Chapters in this work?

I beat the 1st preacher but then died on the next level (the farm). I was put back to the first level (the crypt?) again but it says chapter 2 now.

I’ve run through it but nothing seems to have changed? No extra gear, sin or iron was dropping as I was expecting.

what is the significance of Chapter 2 compared to Chapter 1?
