I don't think you need to kill the Mephit portals actually.

Yeah, you do. The door out of that room is sealed until the mephit portals are killed. I tried to bypass them once and was disgusted to find I couldn't.

Wow, thanks for that list / link.

One thing that surprised me was that about 2/3 of required kills occur in ToB.

Well a full 1/3 of kills are in one mass battle. That aside it's exactly a 50/50 split between SoA and ToB.
The slow blade penetrates the shield.

I don't understand how this is possible. Aren't the vast majority of experience points awarded as the result of killings things? How many can you possibly earn without killing anything and doesn't that leave you unbelievable weak?

You get LOADS of experience from just getting through the game, and there are plenty of non-violent quests to get exp from as well. It might be a bit of a struggle but I can see how it would be possible

Well... if you really want to cheese it, you can earn an unlimited amount of money the moment you step out of Chateau Irenicus then just buy every scroll in sight.

One required kill I noticed you forgot is in the Underdark. To get out of the Underdark, you either have to kill the silver dragon or go through the whole mess in Ust Natha which requires killing the demon the gnomes unearthed and also one of either of these: elder brain, hive mother, or kua toa prince.
