-sama is a mark of ultimate respect which can be used by literally anyone to literally anyone they wanna show a whole lot of respect and/or deference to. It's not a title, it's an honorific meant to honor and that's it.

Many titles do not work as forms of address. For example, you do not call a king "King suchandsuch" to his face. You say "your majesty", or "sir" or "sire". Nor do you address as a duke as "Duke suchandsuch", you say "your grace" or "sir". In fact, that's the same with all noble titles.
Referring to her in the third-person as "Demon Lord Lastina" is absolutely fine (as in that screenshot in your post a few posts back), but directly addressing her as such is cumbersome and unnatural sounding. Dropping the "demon" part should be the first thing to do, and then since "Lady" is the female equivalent of "Lord", anyway...

It's a pretty basic substitution cipher for Japanese, actually.
The highlighted part says (verbatim):
Teriyakibaga
Hanbaga no yaitaniku ni teriyakisosu
wo karametamono

The short line underneath is the price, but I don't know what the numbers are. I assume the one that looks like a 6 is actually a 0, and the last character is "g" (the currency), but I've no idea on the other two.
