D creator Kenji Eno felt that there was a lot of content within the game that could be viewed as excessively taboo for its time (cannibalism, mass murder, excessive graphic violence, etc.) and was concerned it might not be approved for release. So he came up with an 'edited' version of his game, which he showed to everyone and got it approved for publishing. He deliberately waited to submit the master copy to the publishers since he knew the penalty for being late was being forced to deliver the master copy himself.

While Eno was on the plane ride to deliver the master copy, he switched out the 'clean' version of the game with the unedited version. This was how he was able to get his original version of the game published without issues.
